Dear Parents and Athletes,

The Bentonville School District and Northwest Medical Center are partnering to provide
our athletes who participate in sanctioned sports free physicals. This year, both high schools will
be sponsoring a day for you to receive your free physical.
Due to the current Department of Health guidelines and the numerous athletes, you will
be required to follow the AAA COVID-19 guidelines at check in. Instructions will be provided
upon your arrival at your school. The following are the scheduled time frames for each school to
attend physicals:

July 14th
BENTONVILLE HIGH SCHOOL
MAIN GYM ENTRANCE

8am - 10am Fulbright JH

10am - 12pm Washington JH

12:30pm - 3pm Bentonville HS

3pm - 4:30pm OPEN

Masks will be required for every individual attending physicals (masks WILL NOT be
provided if you do not have one). All individuals will also be required to complete a COVID-19
symptom screening form. Please have your athlete wear athletic attire. Please bring a paper
copy of the physical form with the history portion filled out and signed by a parent/guardian.
